OTTY Extra Firm Hybrid Mattress reviewed

The OTTY Extra Firm Hybrid is a 25cm, double-sided hybrid rated 9 out of 10 for firmness on one side and 10 out of 10 on the other, the firmest mattress OTTY makes. OTTY recommends it for people over 15 stone (95kg), and it costs from £524.99 for a single and £674.99 for a double (September 2026).

A flippable build with two firmness levels

The OTTY product page describes larger, thicker encapsulated pocket springs than in its other hybrids, up to 2,000 per mattress, with a different foam stack on each face. You choose the firmness by which side faces up, and because both faces are usable you can also flip it as well as rotate it.

Is firmer better for back pain?

Not automatically. In a randomised, double-blind trial of 313 adults with chronic low back pain, published in The Lancet in 2003 (Kovacs et al.), people given medium-firm mattresses reported less pain in bed and less disability after 90 days than people given firm ones. A 2015 systematic review of 24 controlled trials in Sleep Health (Radwan et al.) reached a similar conclusion, rating medium-firm mattresses best for sleep comfort, quality and spinal alignment. OTTY markets the Extra Firm as orthopaedic, but for back pain on its own the 7/10 Original Hybrid sits closer to what that research found helpful. The case for the Extra Firm is body weight. Heavier sleepers compress a mattress further, and a 9/10 surface keeps the hips from sinking below the line of the shoulders.

Who it suits and who it does not

Back and front sleepers over roughly 15 stone are who this mattress is built for, along with anyone who has found other mattresses too soft. OTTY’s own guidance says side sleepers generally find firm mattresses uncomfortable at the hips and shoulders, and we agree, since at 9 out of 10 there is very little give. The washable cover is a practical advantage over the dry-clean-only cover on the Original Hybrid.

OTTY’s own advice on the page is to be sure you like a firm bed before buying.
